For new church & organization applications, please include a copy of your incorporation, and your constitution and by-laws. We need this information for your file.
If you need any help with any of our forms, including if you would like to make payment arrangement plans, please call our Dallas office at 214-492-1254. Of course, we will be happy to mail and/or fax applications upon request as well.
